Rehabilitation & Community

Stroke rehabilitation is a progressive, dynamic, goal orientated process aimed at enabling a person with impairment to reach their optimal physical, cognitive, emotional, communicative, and/or social functional level.

Rehabilitation is a key component of comprehensive stroke care that is provided in acute and post-acute hospital settings, outpatient programs, and in the community.

Following active rehabilitation, service providers in the community play a key role in supporting the stroke survivor and family to re-engage into the community and adjust to their new normal of living with stroke.
