SJCG-Outpatient Neurology Services
Outpatient Neurology Services provides assessment and treatment to clients living with neurological conditions to improve their overall function and quality of life. Outpatient Neurology Services includes the Neurology Day Program and Neurology Single Discipline Services. Clients can be referred by a Physician or Nurse Practitioner
HCCSS - North West
Home and Community Care Support Services (HCCSS) organizations coordinate in-home and community-based care. They assess patient care needs, and deliver in-home and community-based services to support health and well-being. HCCSS provides access and referrals to other community services, and manages Ontario’s long-term care home placement process
